I authorize Vida Property Management LLC, its owners, agents, and authorized screening providers to contact the current and former housing providers I identify and to request and receive factual information reasonably related to my tenancy, including dates of occupancy, rent amount, payment history, outstanding balances, lease compliance, notices, authorized occupants or pets, care of the property, and eligibility for re-rental. I authorize those housing providers to release that information to Vida for evaluating my housing application. This authorization does not request medical information, disability or accommodation information, or other protected-class information. A third party’s failure to respond is not automatically treated as negative housing history.
